• To join our plan, simply download the forms above, fill them in, sign and send them in to us! (see options below on how to send them in). 
   

• If you want to add a child / children under the age of 18 to your plan, download additional form/s from the "Add a child" link above.
   

• For each additional adult you would like to insure (such as your spouse), it is necessary to print out an additional set of forms.
   

• Take the amount of days that you (and those you are insuring with you) will be staying in Israel, and multiply that by the appropriate rate. To view the insurance rates, click here. (For example, if you're staying in Israel for 180 days, multiply 180 X $1 = $180). Click here for a site that will help you calculate the exact amount of days between two dates.

• IMPORTANT: Please put your e-mail address on the forms so we can contact you if there is a need.
   

• On the payment form, please specify for whom you are paying and how much the total amount comes out to be.
   

• Payments for a full year may be divided into 6, or 3 payments for a half a year. Please specify if you want to pay in multiple payments. Please note however, that we often have difficulty processing payments from over-seas credit cards. If we do not succeed in processing multiple payments, we will need take the full payment at one time. If this is a problem for you, please pay with a few checks instead.
   

• If you will be paying by check, please write it out to: "Ayalon Insurance Co" and mail it to the address below (in option 3).
   

• Please note that if you pay by check, we cannot begin your insurance coverage until the check arrives in the mail.
   

• How do you get the forms to us? (There are three options).

• Once the forms are received by our office, it takes only about 24 hours to process them. Once they are processed, you receive a number in the computer and are insured. The number is then printed on a temporary card and mailed out to you. You should receive it within a few days. A Magnetic Personalized card is then prepared by Leumit and mailed out to you within 2-3 weeks.
   

• Once you are signed up, make sure to download our information sheets. Print them out and keep them handy at all times, especially in case of emergency.

• Please do not send in the forms more than 2 weeks before the start of your coverage. Ayalon will require a new "Health declaration" for forms sent in too early.
